The resources listed below are for individuals or organizations who want to help get the word out in their communities: hand out flyers or application forms in your neighborhood or at gathering places like community centers and libraries.

This flyer is a checklist for consumers to prepare for the transition. 

DTV Transition flyers in more than 50 additional languages are available from the DTV Answers website - you will also find contact information for ordering braille flyers.

Coupon Request Forms

Coupon request forms are currently available in English, Spanish, Cambodian, French, Hmong, Korean, Laotian, Russian, Simplified Chinese, Tagalog, Traditional Chinese and Vietnamese.

Residents of licensed nursing homes, intermediate care facilities, or assisted living facilities must submit a different form to apply for a coupon. A family member, friend, or facility administrator may complete and submit the application on behalf of the resident. Facility administrators may apply for coupons on behalf of more than one resident, following the instructions on the application. Nursing home or care facility residents cannot apply by phone or online.
